If your institution requires IRB approval, please provide the relevant information in those fields. If there are other investigators on your team, you will need to enter their information as well. Our Data Sharing and Publications Policy can be found here, and our LEADS Data Use Agreement can be found here. Please review the document. Your signature on this request form and on the LEADS Data Use Agreement confirms your agreement to adhere to the policy.
Currently, only baseline data is available for sharing with external investigators. Please ensure your project does not overlap with the primary aims of the LEAD Study.
